This policy explains the use of your personal information, and the reasonable efforts we make to protect your information and what choices you have. We may update this policy from time to time on our Site. Please refer back to this site regularly for updates.

Scope of Policy. This policy applies to your use of the LAC site. This policy does not apply to your use of unaffiliated sites to which this site only links.
Personal identification information. We will request personal identification information from you (your name, age, contact information, address, email, phone, credit card information, etc.)in connection with our company services for our business purposes and in connection with other activities, services or resources we make available on our Site. We collect this information from you that you voluntarily submit. We will reasonably endeavor to never intentionally disclose any personal identification information about you to any third party without having received your permission, except as provided for herein or as permitted or required under law. We will share personal information with service providers who are independent contractors or trusted service providers that need access to your infomraiton in addition to our employees to provide operational or other support services or we may share statistical information with other third parties . We may also provide information to regulatory authorities and law enforcement officials in accordance with applicable law or when we otherwise believe in good faith that the law requires it. We will collect your identification information without limitation through the use of the following types of methods: Use of Cookie technology, which is an element of data that a website sends to your browsers which may then store it on your system; IP address tracking when you are on the internet, when you review our website or request pages from our site, our servers log your IP address; and Web beacons; which is a small graphic image on a webpage or document that a website can use to determine information about a user.
Non personal identification information will also be possibly used, such as the browser you use, your type of computer, and other technical information. Our website and operations system may gather this non personal information to assist us with our marketing efforts for our business, and to administer our website appropriately, so that we may continue to improve our communication with our clients and our website functionality
Privacy of Children.
Our website is intended only for adults over the age of eighteen years of age. We recommend that all adult guardians or parents restrict their use of the internet to direct them only to websites that are directed to children, and not to any online website that may collect their personal information. Our registrations are limited to adults over 18 and any minor child, must be registered by their legal guardian or legal parent if they become a member of LAC. In the case of any minor becoming a member of the LAC, it is agreed and understood that the parent or legal guardian will be present and monitor any viewing of LAC website content and give consent to their child to view it.
Links to other sites.
Users will or may see content on our site that links to other sites and services of other third parties. We do not control the content or links that appear on these sites. Those third party sites and services should have their own privacy policies and customer service policies and we do not monitor their polices. You should review the privacy policies of any third party sites or services before providing your personal information to those sites.
Choice/Opt-in and Opt-out
By using our website and becoming a member of LAC you are consenting to Opt-in to our policies. If you later change your mind, you will need to change your option by contacting us at LAC on our website on our customer service link or contact Amanda Anderson, of LAC at the address below. It is not always possible to completely remove or change information in our database and servers once you have provided it, but we will make a reasonable effort to do so upon your request. We are unable to have your information removed from the records of any third party who has been provided with your information in accordance with this policy.
Targeting advertising. We may also use certain programs offered by Adobe, Facebook Linkedin, Yahoo, Bing or similar advertising programs to help deliver ads that we believe are relevant to your interests in the future. We will update this privacy policy as we intend to disclose to you the customization of the LAC website which may effect our privacy policies.

Get a list of those with whom we’ve shared information
- You can ask for a list (accounting) of the times we’ve shared your health information for six years prior to the date you ask, who we shared it with, and why.

Example: A doctor treating you for an injury asks another doctor about your overall health condition.
Run our organization
- We can use and share your health information to run our practice, improve your care,

Bill for your services
- We can use and share your health information to bill and get payment from health plans or other entities.

Help with public health and safety issues
